How to Apply

Complete the online application form (found here).

Upload the following materials:

  • Personal Statement (PDF format)
    • Personal Statement Guidelines:  In no more than three typed, double-spaced pages, describe your reasons for pursuing a career in healthcare.  Include any related experience you have and explain how that experience has informed your career goals.
      • Note:  Please make sure your name and current date are clearly stated on your personal statement.
  • Two Letters of Recommendation
    • The recommender should be someone that knows you well enough to give an informed opinion of your motivation, intellectual ability, capacity for sustained effort, and person qualifications.  We suggest that at least one of the two letters be an academic reference.
    • On page 4 of the application you will be asked to enter your recommenders’ information along with their email address. Once your application is submitted we will email them a link to enter their recommendation. Reminders will also be sent out to both the recommender and applicant if recommendations are not received within a week to 10 days of request.
  •  All Unofficial Transcripts (PDF format)
    • Unofficial transcripts for all undergraduate and graduate work completed or in progress.  You need to provide a transcript for any college-level work you have completed or attempted. This includes all transfer, international, and study abroad credit.
      • Note:  Your transcripts need to include the name of the institution and your name.
  • Application fee:  $50 (payment by credit or debit card)
