APPLICATION INSTRUCTIONS
In order to be considered for admission into the program, please read and carefully follow the instructions below. Incomplete applications will not be considered. A complete application consists of two parts: the online application form and a packet containing your supplementary material (2 reference letters and official transcripts).
STEP ONE: Complete the online application form; before you are able to submit the application you will be required to attach a resume and a personal statement. These documents can be attached as either PDFs or Word documents.
Personal Statement Guidelines
In the Personal Statement section of the application, in no more than one typed, double spaced page, describe your reasons for wishing to pursue a career in speech language pathology. Include any relevant experience you have and explain how that experience has informed your career goals.
STEP TWO: Mail your supplementary material.
After you have submitted the online application form, you will need to mail in the rest of your application material. All material should be sent in a single packet, addressed to:

Your packet should include all of the following:
- Official, sealed transcripts for any and all undergraduate and graduate work completed or in progress. You need to provide an official transcript for any college level work you have completed or attempted. This includes all transfer, international and study abroad credit. Transcripts should be issued to you (the applicant) and then mailed to us in the original, sealed envelope with the rest of the supplementary application material.
- Two letters of recommendation from people who know you well enough to give an informed opinion of your capacity to work with children, motivation, intellectual ability, capacity for sustained effort, and personal qualifications. We suggest that at least one of the two letters be an academic reference. These letters must come to us in one of two ways: Via regular mail, written on letterhead, signed and sealed in an envelope with the person who recommended you writing their name across the flap; or, via email, attach the letter written on letterhead and e-signed by the person who recommended you.
